For the authorization regarding the online betting:
1. the application form regarding the activity authorization. The application will be drafted and typed in the Romanian language, signed and stamped by the legal representative of the economic agent and will include: the applicant identification data, the type and series of the gambling equipment to provide the support for the organization, exploitation and transmission of the gambling games that are the object of the application form, the location of this equipment. If decided on using the electronic equipment, the application form will solely be compiled as such;
2. the documents attesting to the fulfilling of the decision requirements;
3. the functional regulations for each game, where it will be mentioned, as case may be, the percentage given to the winnings from the total turnover;
4. the internal regulations concerning the activity organization and development, with the following details: staff duties and responsibilities, the financial-accounting documents, method of providing the security of the documents, of the information and data, the means of inter-connecting with the system of the monitoring and reporting operator, as well as the server location, the after-disaster recovery plan, the backup system of the database in use;
5. the documents regarding the ownership status or the right of user upon the gambling means and of the technical equipment, which provides the support for the organization, exploitation and transmission of the gambling games that are the object of the application form;
6. making available the monitoring contract concluded with the monitoring and reporting operator, for its approval;
7. the confirmation issued by the monitoring and reporting operator regarding the auditing upon the technical equipment, which provides the support for the organization, exploitation and transmission of the gambling games that are the object of the application form;
8. the document issued by the monitoring and reporting operator where it is stipulated that the gambling schedule, the theoretical pool of prizes and the generator of the random numbers are internationally certified by bodies/labs that are acknowledged in this field;
9. the proof for the guarantee fund;
10. data concerning the contact person assigned at the management level for providing conformity with the authorization criteria;
11. an affidavit from the gambling organizer that the space/s where the gambling equipment will be installed and exploited for providing the support for the organization, exploitation and transmission of the gambling games fully complies/y with the sanitary norms regarding the fire proof regulations, in case they own technical equipment on the territory of Romania;
12. for live broadcast, the proof of the fact that the location where the live transmission is made via the internet-type communication systems, hard cable or mobile telephone, holds the authorization for the indoor bingo-type games exploitation or a valid authorization of the bingo games organized via the television network systems;
13. a document that will confirm the right of user granted to the organizer by the software rightful owner;
14. the proof of registering as a personal data operator with the National Authority for the Supervision of Personal Data Processing.
