Documents required for obtaining the gambling exploitation authorization

For the authorization regarding the indoor bingo games:

1. the application form regarding the activity authorization.  The application will be drafted and typed in the Romanian language, signed and stamped by the legal representative of the economic agent and will include: the applicant identification data, the type and series of the gambling equipment that is the object of the application form, the gambling spaces for the intended activity, as well as the business hours.  If decided on using the electronic equipment, the application form will solely be compiled as such;
2. a detailed description of the activity, including a floor plan of the gambling spaces, with the exact location of the main and the auxiliary gambling equipment;
3. the functional regulations for each game, where it will be mentioned, as case may be, the percentage given to the winnings from the total  turnover;
4. the documents regarding the ownership status or the right of user, as well as the proof of having had the gambling space registered as a working point;
5. the internal regulations concerning the activity organization and development, with the following details: staff duties and responsibilities, the financial-accounting documents, method of providing the security of the documents, of the assets, etc.;
6. the proof of the equipment technical inspection of the turbo-blower, issued by the Romanian Office of Legal Metrology;
7. the documents to attest to the legal origin of the main and the auxiliary gambling equipment;
8. the proof for the guarantee fund;
9. an affidavit from the gambling organizer that the space where the gambling equipment will be exploited fully comply with the sanitary norms regarding the fire proof regulations.

For the authorizations of exploitation of the fixed odds betting gambling:

1. the application form regarding the activity authorization.  The application will be drafted and typed in the Romanian language, signed and stamped by the legal representative of the economic agent and will include: the applicant identification data, the type of the activity that is the object of the application form, as well as the gambling spaces for the intended activity.  If decided on using the electronic equipment, the application form will solely be compiled as such;
2. a detailed description of the activity, including the location where the server is installed.  There will be mentioned the manner of inter-connectivity with the bookmakers, how to guarantee the security of the centralized information regarding the cashflow for each bookmaker, as well the manner of inter-connectivity with the system at the Department of Public Finances;
3. the functional regulations for each game, where it will be mentioned, as case may be, the percentage given to the winnings from the total  turnover;
4. the documents regarding the ownership status or the right of user for the gambling spaces (it only applies to the working points that are not included in the certificate of good standing issued by the Trade Registry);
5. the internal regulations concerning the activity organization and development, with the following details: staff duties and responsibilities, the financial-accounting documents, method of providing the security of the documents, of the assets, as well as the server location;
6. a document to attest to the right of user granted to the organizar by the lawful holder of the IT software;
7. the proof for the guarantee fund;
8. an affidavit from the gambling organizer that the space where the gambling equipment will be exploited fully comply with the sanitary norms regarding the fire proof regulations.
9. for adding new bookmakers:

a)      the application form regarding the registration of the new bookmakers.  The application will be drafted and typed in the Romanian language, signed and stamped by the legal representative of the economic agent and will include: the applicant identification data, the location of the agencies and terminals inter-connected in the system within them.  If decided on using the electronic equipment, the application form will solely be compiled as such;

b)      the manner of inter-connectivity with the reference server of the system;

c)      the proof for the guarantee fund;

d)      an affidavit from the gambling organizer that the space where the gambling equipment will be exploited fully comply with the sanitary norms regarding the fire proof regulations.
